New Students to SAS
Student Accessibility Services (SAS) facilitates access to university programs for students with disabilities or medical conditions by providing accommodations and additional resources including learning skills, assistive technology training, and advocacy support.
Because arranging academic accommodations can take time, we encourage you to connect with our office early as possible.
The waiting time for an initial meeting is approx. 6-8 weeks.

Step 1: Complete the Student Information Form
If you are a student with a disability, or suspect you have a disability, complete the student information form below to begin the accommodation process. You will be contacted via email for your initial meeting with the SAS Coordinator. Please do not complete this form more than once.

Step 2: Submit medical documentation
While you are waiting for your initial meeting, please have your registered health professional completed this Medical Questionnaire.
If you have a diagnosed learning disability, a psycho-educational assessment report will be required.
If you do not have documentation, this will be discussed during your initial meeting with the SAS Coordinator.
Step 3: Meet with the Student Accessibility Coordinator
The SAS Coordinator will meet with you to review your documentation and discuss your accommodation and learning needs.
Once accommodations are determined, the SAS Coordinator will email your and your professors a copy of your approved accommodations in the form of an accommodation letter.
All information about your disability or medical condition are confidential between you and SAS.
The SAS Coordinator may refer you to the SAS Specialist for learning skills and/or assistive technology supports, or to other applicable campus resources.
