To help you navigate the accommodation process, here are the forms associated with the entire process.

Upon medical instruction and advisement, when you need an accommodation, complete an Accommodation Request form.

If your accommodation request involves a support person or service animal, complete an Accommodation Request ~ Support Person/Service Animal form.

As a supervisor, this checklist, the Accommodation Planning Checklist, will help guide you through the accommodation process.

An employee requesting an accommodation is expected to work together with their supervisor/manager/unit head to create their Workplace Accommodation Plan.
